DUTIES AND RESPONSIBILITIES
Assists in completing client intake forms and assessments.
Obtains medical and psychological evaluations
Assists in writing Individual Service Plans (ISP).
Assists in writing skills and behavioral programs for clients and in data collection.
Teaches skills to individuals that are based on the ISP and records individuals' progress in activity notes.
Maintains contact with the individuals' families.
Applies for Supplemental Security Income (SSI) and Social Security (SS) benefits and files reports.
Completes monthly reports and other assigned paperwork.
Monitors medication logs and self-administration of medicine, if applicable.
Serves as on-call case manager as scheduled. In this capacity, he/she may be asked to operate the lift to assist non-ambulatory individuals in performing daily living skills.
Provides related services to individuals served:
Transportation (may involve operation of lift-equipped van)

Budgeting and money management
Completes training on all aspects of Regional MIS System as it pertains to individuals' basic information, treatment plans, and progress notes; enters all in the system in a timely manner.

Responsibilities:
Conduct initial/ongoing assessments to meet the needs of the person served

Conduct training/in-service training, annually, and as needed to ensure staff and other team members are competent on the implementation of the program plan and/or documentation sheets
Provide follow-up interviews/home visits/observations for continued support for the individual's needs
Be flexible/adaptable to ever changing policies/procedures/status/issues related to the individual in order to support him/her
Have effective record keeping (i.e. monthly/quarterly notes; in-service training; contact information; program plans) to ensure information is readily available to interested parties (i.e. management; surveyor)
Act as liaison with the public and mental health community for the purpose of supporting individual development goals
Assist in developing procedures for training materials to enhance programs for individuals and ensures program operations are in compliance with established guidelines
Continued education on behaviorally based interventions & techniques to stay competitive in the field
Follow-up visits and monthly/quarterly notes of individual's progression
Perform other related duties as assigned for the purpose of ensuring the efficient and effective functioning within the work unit
Qualifications:
Must have experience with behavioral intervention, and/or a strong educational emphasis on the principles of learning and developmental psychology

The Palliative Care Counselor (PCC) is responsible for providing holistic care through counseling and psychosocial support for patients and their families living with a serious life limiting illness or injury seen by the palliative care team. The PCC can work effectively within an interdisciplinary team. The PCC will assist the patient and their family unit in supporting and addressing their psychosocial needs per the palliative care interdisciplinary team in which the PCC is a core team member. The PCC completes psychosocial assessments, develops care plans for providing psychosocial education and emotional support to patients, families and caregivers on nonpharmacological symptom management, psychological stressors to anxiety, depression and pain. Provides individual coping skill strategies. The PCC will assess complicated family dynamics (financial, culture, anticipatory and complicated grief), intervene in crises and mediate conflicts within families. The PCC will be responsible for identifying correct legal next of kin, researching power of attorney for health care, guardianship, and completed advanced directives in preparation for scheduling and attending patient-family centered goals of care conference meetings. In addition, the PCC will work with the palliative care chaplain and social workers to plan and prepare family units for bereavement support. This position reports to the manager of the palliative care department.

Job Description
This position has a designated special entry salary that is non-negotiable.
Shift differential supplemental pay is offered for 2nd, and 3rd, and weekend shifts.

Under supervision, learns to provide investigative and comprehensive case management for a caseload involving child abuse or neglect.
Roles and Responsibilities
Initiates timely, face-to-face responses to screened-in reports of alleged child maltreatment and conducts assessments that identify and address safety concerns.
Addresses identified safety concerns by immediately developing and implementing safety plans.
Conducts diligent search efforts to locate the child/family when unable to make contact.
Implements Solution Based Casework (SBC).
Partners with child(ren) and parents in the development and utilization of the Family Functioning Agreement, Action Plan, and Family Plan to achieve desired individual and family-level outcomes.
Completes genograms to identify available family support network.
Collaborates with community partners to create systems of support for children and families.
Ensures that frequency and quality of visits with children and families are adequate to ensure safety, permanency, and well-being.
Identifies the physical, educational, and mental/behavioral health needs of children, parents, and foster parents and ensures identified needs were effectively addressed.
Establishes permanency goals for the child(ren).
Ensures foster care placements and/or living situations are stable and in the best interests of the child(ren) and are consistent with achieving established permanency goal(s).
Makes concerted efforts to achieve permanency for children in a timely manner and consistent with regulations contained in the Adoptions and Safe Families Act.
Ensures that siblings in foster care are placed together unless a separation is necessary to meet the needs of one of the siblings.
Maintains child's connections to his or her neighborhood, community, faith, extended family, tribe, school, and friends.
Promotes, supports, and otherwise maintains a positive and nurturing relationship between the child(ren) in care and their biological parents/caretakers by encouraging and facilitating activities and interactions beyond visitation.
Documents case management activities within time frames established by policy.
Celebrates positive outcomes with child(ren) and family.
Utilizes privately owned vehicle for work-related travel and duties (including transportation of children).

* Intervenes as appropriate to manage resource utilization and plan of care.
