This position will provide information, education, and support to patients, families, and caregivers regarding end-of-life planning and care. This role will assist them (patients, families, caregivers) in the transition to Hospice care when the patient is both eligible and appropriate for Hospice services.
Hospice Journey Counselor Responsibilities:

Summary:
